CDPH HAI Antimicrobial Stewardship Program (ASP) Honor Roll Instructions and Criteria​

Instructions

  1. Review criteria in the tables below to determine your institution’s qualifications for Bronze, Silver, or Gold designation and the appropriate application type.
  2. Collect the minimum documentation required for all designations.
  3. Request the link to your facility specific application using the following link: CDPH HAI ASP Honor Roll Application Link​​. If you do not receive the link after 30 minutes, please check your spam folder. If the link is not in your spam folder, reach out to HAI_AS@cdph.ca.gov. 
  4. Complete and submit the online RedCap application by the enrollment deadline. Your answers and document uploads will be saved automatically. If you need to complete your application at a later date, please click “Save & Return Later” when exiting the application and follow the prompts. If multiple people will be completing the application, please share the link to login. 

After Submitting Your Application

  1. You will automatically receive an email confirmation with a copy of your application for your records. Please review the application carefully to confirm you have submitted all documentation as we will NOT be contacting facilities for missing documentation. 
  2. If you notice that documentation is missing, please email it to HAI_AS@cdph.ca.gov. 
  3. The review process takes approximately 12 weeks and consists of three stages, including external reviewers for Part III. Notification emails of designation results will be sent approximately 12 weeks after the application deadline. ​​​

Criteria for Bronze, Silver, and Gold 

Refer to the Minimum Documentation​, Demonstration of Outcomes​, and Community Engagement pages for corresponding documentation and requirements. All information, including documentation, is submitted within the RedCap online application.

​Bronze
 ​Leadership 
  • Provide support and time for dedicated personnel to manage the stewardship program and conduct daily stewardship interventions.
  • Provide resources to effectively operate the program. 
Accountability 
  • Appoint a program leader(s) with antimicrobial stewardship (AS) training who is responsible for AS activities.
  • Collaborate with other departments and groups on AS activities.
  • Pharmacy (and/or Drug) Expertise 
  • Appoint a pharmacist, ideally as the co-leader of the stewardship program, to help lead implementation efforts. 
Action 
  • Create antimicrobial stewardship policies to guide program activities.  
  • Perform prospective audit and feedback (formal review of active antibiotic therapy by the AS team with recommendations on optimizing therapy if appropriate) or preauthorization (approval required by the AS team for specific antibiotic agents). 
  • Develop facility-specific treatment guidelines that include antimicrobial recommendations for common infections. 
Tracking 
  • Publish an antibiogram. 
  • Monitor antimicrobial use (e.g. days of therapy/1000 patient days) with facility-, individual-, unit-, or service-specific antimicrobial use (AU) reports. 
  • Track prospective audit and feedback intervention outcomes (e.g., acceptance rates and types of interventions). 
  • Monitor adherence to facility-specific treatment guidelines for common infections. 
Reporting 
  • Share antimicrobial stewardship reports (e.g. facility-wide and/or prescriber-specific antimicrobial use) with leadership and prescribers. ​
Education 
  • Educate providers while doing prospective audit and feedback. 
  • Provide competency-based training and education.
