Application Requirements for Manufacturing with Industrial Hemp

Thank you for your interest in an Industrial Hemp Enrollment and Oversight authorization (IHEO authorization). The below forms are required along with the associated payments in order to begin processing your application(s).

Please note: If you manufacture multiple Industrial Hemp (IH) commodities, please download and complete all forms associated with those commodities. For example, if you manufacture extracts and use those extracts in your pet food, you would need to fill out all forms under "Extract Manufacturer" and "Processed Pet Food Manufacturer." Please ensure all forms are filled out completely, with an authorized signature and submitted with the proper payment listed in the application(s).

New Applic​ants: Please fill out the forms below and remit with payment to the address listed on the instructions page of each application that is applicable.

Existing Licensees​/Registrants: If you have an existing license or registration, you will need to follow the same process as listed above. If approved for IHEO authorization, please note: Your current expiration date will change to match the ​same date as your IHEO authorization.
