Important Notice

Renewal Application Fee: Effective January 1, 2023, Laboratory Field Services has reinstated the renewal application fee to $975.00 for Tissue Bank licenses that expire on or after January 1, 2023.

New Application Fee: Effective January 1, 2023, the new application fee has been reinstated to $975.00.

The California Department of Public Health LFS Tissue Bank Section licenses and surveys several types of facilities:

1. Cadaveric tissue: collection, processing and distribution facilities.

2. Cadaveric tissue: storage and patient distribution facilities.

3. Assisted Reproductive Technologies facilities including sperm banks.

4. Stem cell processing from sources other than cord blood and circulating blood.

5. Donor milk collection, processing, storage, and distribution.

6. Autologous tissue stored overnight or longer.

7. Any facility described above which is located in California or is located outside of California and wants to operate in California.
